Middle Park Medical Center providing services for the Veteran’s Choice Card program

Middle Park Medical Center (MPMC) has recently been accepted into the Veteran’s Choice Card program and is now able to see eligible Veterans at their Kremmling, Granby and Walden facilities. “It gives us great honor to be able to give back and care for the Veterans who have put their lives on the line for our great country,” said David Ross, CEO of MPMC. “This program is a vital lifeline to our heroes who deserve the absolute best care.”

The Veterans Access, Choice, Accountability Act (VACAA) of 2014 is a law that expands the number of options Veterans have for receiving care to ensure timely access to high-quality care. The VACAA allows eligible Veterans who live more than 40 miles from a VA facility, or are unable to get a VA appointment within 30 days of their preferred date, or within 30 days of the date determined medically necessary by their physician, to obtain approved care in their community instead.

It is said that as much as 10% of Grand County’s population is made up of Veterans, the largest percentage of any county in the state. Include that number with the Veterans that live in Jackson and Northern Summit counties and the importance of care provided at MPMC for our service men and women is immeasurable. MPMC will be working closely with Duane Dailey, Veterans Officer for Grand County, along with his counterparts in Jackson and Northern Summit counties to coordinate the most accurate and up-to-date information for Veterans.

In order to verify if you will qualify for the Choice Card program, you must call 866-606-8198 for authorization. Before calling, the following criteria will need to be reviewed:

  • Have you been told by your local VA facility that you will need to wait more than 30 days from your preferred date or the date medically determined by your physician? OR
  • Is your current residence more than 40 miles from the closest VA health care facility? OR
  • Do you need to travel by plane or boat to the VA medical facility closest to your home? OR
  • Does a geographic challenge, such as extensive distances around water or other geologic formations, such as mountains, present a significant travel hardship

Once you have qualified through the Choice Card Program, you will be able to schedule an appointment with one of MPMC’s providers and receive the care that you deserve
