At your next medical appointment, please bring a valid form of identification. You will be given an activation code to sign-up for the new Patient Portal.
If you’d like to sign-up for the new Patient Portal prior to your next medical appointment, please go to your nearest MPMC clinic. Please provide the front desk staff with a valid form of identification. Then they can provide you with an activation code.
For patients with login credentials to our prior patient portals, the old portals will remain open until April 15, 2018.
